
According to an FRSC report obtained on Tuesday, the accident involved four vehicles — a red Nissan Micra, a white tanker, a truck and a commercial Toyota Hiace bus — and was caused by brake failure.

The report stated that one of the drivers lost control, triggering a chain collision. The female victim died on the spot, while other occupants sustained varying degrees of injuries and were rushed to a nearby hospital for treatment.
The Lagos-Ibadan Expressway, particularly the Iwo-Ojoo axis, has remained a hotspot for fatal crashes, with several recent incidents linked to brake failure, loss of control and collisions involving heavy-duty vehicles.
